- Solicitation VA-244-08-RP-0316 Amendment VA004Project: Remodel Diagnostic Service Nuclear Medicine Camera Proposal Due Date is extended to March 27, 2009 at 2 PM EST. Contractor question: REQUEST FOR INFORMATION - POC'S On electrical drawing E-3 it shows the power for AHU-NUC-1 coming from the existing switchgear in the basement electrical room 127. It states that we need to install a new 225A breaker and that the type and interrupting rating shall match existing but we don't know what the existing is. We would like to know if we could get the make and model number of the existing switchgear and breaker to aid in pricing the correct breaker. Answer from A/E: Date: March 10, 2009 The circuit breakers in the existing switchgear are Square D circuit breakers. Sample ( MEF436LI ). REQUEST FOR INFORMATION - POC'S Drawing A-2 has quite a few errors: "Room 2171 is shown twice on the Finish Plan (Injection Room & Camera Room) "Dimensions are not legible in the Finish Plan "Sheet Vinyl in camera rooms w/strips & squares???? "Door Schedule - door Number 2171A? "Door Schedule - door Number 2172 Injection Room? "Finish Schedule - Room No. 2179 Camera Room? "Finish Schedule - Room No 2171 Camera Room? "Finish Schedule - no finish schedule for Room No 2170 Camera Room? "No Finish Schedule for Room 2171 Injection Room? Please verify/clarify. Answer from A/E: Date: March 13, 2009 PROJECT: Nuclear Medicine Louis A. Johnson VAMC Clarksburg, WV VAMC Clarksburg Project 540-05-270 RMP Project 07039 THIS ADDENDUM, consisting of two (2) Pages, forms a part of the Contract Documents and modifies the original Bid Documents dated 23 June 2008. Acknowledge receipt of this Addendum on the Bid Form. Failure to do so may subject the Bidder to disqualification. CLARIFICATIONS: 1. No finishes are assigned for the Injection Room with the exception of miscellaneous painting that may be necessary due to the course of the work, primarily HVAC. If necessary, touch-up paint to match adjacent surfaces or provide P1 for construction item requiring repainting. 2. To the best of our knowledge, dimensions shown on the Finish Plan are legible. 3. In response to Contractor Request for Information, we are verifying that patterning indicated on Finish Plan 1 /A-2 consists of squares and rectangles. 4. Door 2171A as listed in the Door Schedule is confirmed as shown. 5. Room 2170 CAMERA ROOM as listed in the Finish Schedule is confirmed as shown. 6. See below for correction in Finish Schedule regarding Room 2170. CHANGES TO DRAWINGS: 1. Sheet A-1 - PLANS: A. Add door #2171A to indicated sliding pocket door between Camera Rooms 2170 and 2171. 2. Sheet A-2 - FINISH PLANS/ELEVATIONS: A. At "INJECTION ROOM" delete "2171" room number and insert "2172". B. Finish Schedule, Room indicated as "2179 CAMERA ROOM". Delete "2179" and insert "2170".
