Description

This is a SOURCES SOUGHT NOTICE ONLY. This is not a request for proposals or quotes. This document does not obligate the Government to award a contract. The purpose of this document is not to provide notice of the Government's intent to post a future solicitation. Responses to this announcement will be used by the Government to make appropriate acquisition decisions. After the responses to this announcement are reviewed the Government may issue a Request for Proposals solicitation for perfusion services under NAICS 62199. All interested offerors will have to respond to the formal solicitation announcements in order to be considered eligible for selection. The Department of Veterans Affairs Salt Lake City Health Care System, Utah is seeking sources for perfusionist services to treat patients of the Veteran's Affairs Hospital. Perfusionists must hold a degree in health science education and must hold one of the following certifications: RN, CCP, MLT, EMT, LPN, or CNA. The hospital requires a perfusionist who can perform all aspects of the autotransfusion in the intraoperative and perioperative settings. This perfusionist must be able to provide these services on a 24 hour as needed basis. If called to the hospital after hours, the perfusionist must be at the hospital within one hour of notification. The perfusionist will also be responsible for monitoring the autotransfusion equipment when it is in use. Perfusionists must have knowledge of the equipment and supplies needed to assist in performing a cardiopulmonary by-pass, knowledge of intra-aortic balloon pumps, and knowledge of blood sparing equipment. If you are interested and capable in providing the required services please provide the following information. Responses to this notice should include: company name, address, and phone number. Please also include your business size pursuant to the following questions: 1.) Is your business small; 2.) If small does your firm qualify as a small disadvantaged business; 3.) If disadvantaged please specify under what group and if you are Section 8a certified; 4.) Is your firm a certified "Hub zone" firm; 5.) Is your firm a woman-owned business; 6.) Is your firm a certified Service Disabled Veteran Owned Small Business, or a Veteran Owned Small Business. Responses to this notice shall be emailed to the attention of Adina Torberntsson, Department of Veterans Affairs at adina.torberntsson@va.gov. Responses must be received no later than 5:00PM MST Friday June 22, 2012.
