- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation is issued as a Request for Quotation (RFQ) and the reference number is IHS1396634.� This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 05-77 and the Health and Human Services Acquisition Regulation (HHSAR).� The Offeror(s) must quote on an all or none basis. Written quotes are required (oral offers will not be accepted). Award will be made using Simplified Acquisition Procedures in accordance with FAR 13.106-2. Please include CAGE Code, DUNS, Tax Identification Number, FOB Point, Discount Terms and Delivery Date. The NAICS Code is 339112 Size Standard is $ 1,000 employees. The enclosed solicitation is set aside for any small business concerns. 1.�� GENERAL: The Phoenix Indian Medical Center currently has a need for annual preventive maintenance, repairs, parts replacement (as required), upgrade software, un-schedule repairs, emergency repairs include after hours (4:30pm � 8:00am), replace filters, labor and travel for Sterizone Sterilization system VP4 and ON2 Oxygen Concentrators located in Central Sterile Room. The agreement shall be a full service. The medical equipment are located at the Phoenix Indian Medical Center (PIMC) in Phoenix, Arizona.� The PIMC is located at the address below: ������� 1.1 ������� Phoenix Indian Medical Center �������������������� 4212 N. 16th Street �������������������� Phoenix, Arizona 85016 2.�� SCOPE OF WORK: The contractor shall provide all supervision, labor, parts, materials, tools, supplies, maintenance, ground transportation, and equipment service for Sterizone Sterilization system VP4 and ON2 Oxygen Concentrators. ����� 2.1� Contractor shall repair or replace all components necessary to return units to Original Equipment Manufacturer�s specifications. 2.1� Contractor shall warrant all services performed and units shall remain defect free 30 days from the date of service/maintenance/repair acceptance. 3.���� EQUIPMENT DESCRIPTION: 1 Sterizone Sterilization System -VP41316005 -Sterizone VP4 2 Sterizone Sterilization System -VP41316018 -Sterizone VP4 1 ON2 Oxygen Concentrator -18770 -OC-03 2 ON2 Oxygen Concentrator -18740 -OC-03 See Attached SOW
