Description

Appended to answer questions asked of the original Solicitation. See attached document S05 36C020Q0406 0001. The Department of Veterans Affairs (VA) Great Lakes Consolidated Mail Out-Patient Pharmacy (GL-CMOP) located at 5th and Roosevelt Building 37NW, Hines, IL 60141 is a provider of outpatient pharmaceutical services that dispenses approximately 100,000 prescriptions daily to Veterans homes. To support the daily operations, GL-CMOP needs a Service Provider to haul away Non-hazardous waste and recyclable waste from the facility. Non-hazardous waste and recyclable waste shall be picked up in a timely basis and dumped at an EPA approved landfill. Non-hazardous, solid-waste and recyclable commodities collection at the Great Lakes CMOP in Hines, IL. See attached statement of Work for additional information. Period of Performance: 1 October 2020 - 30 September 2025
