- Page 4 of 4 AMENDMENT #3 TO REQUEST FOR QUOTE (RFQ): 36C25820Q0306: This notice is an amendment to RFQ 36C25820Q0306 for Ambulatory Cardiac Monitoring Services. The required services are to be performed for the Phoenix VA Healthcare System located in Phoenix, Arizona. This amendment clarifies why FAR Clause 52.219-4, Notice of Price Evaluation Preference for HubZone Small Business Concerns (Deviation 2019-01), was incorporated by the previous amendment into this RFQ. Incorporation of this clause does not imply this requirement has been set-aside for HubZone. This clause is required to be in the RFQ and the subsequent contract for requirements that have been competed under Full and Open Competition (Unrestricted). Since, this requirement is an Unrestricted competition, Block 10 of the SF 1449 for the RFQ has the Unrestricted box checked as the type of acquisition.
