Page 2 of 2 The Department of Veterans Affairs, VISN 08, Malcom Randall VAMC, (NF/SGVHS) located at 1601 S.W. Archer Road, Gainesville, FL 32608-1197 intends to solicit quotes from contractors to provide NF/SGVHS services under a Firm-Fixed Price (FFP) contract. The quoter shall provide Perfusion/Autotransfusion Services to eligible beneficiaries of the NF/SG VHS. The Contractor s physician(s) care shall cover the range of Perfusion/Autotransfusion Services as would be provided in a state-of-the-art civilian medical treatment facility and the standard of care shall be of a quality, meeting or exceeding currently recognized national standards as established by the American Board of Cardiovascular Perfusion http://www.abcp.org The quoter shall propose a minimum of four (4) key personnel to be credentialed and be available for scheduling to meet the requirements of the contract. The VA Full Time Equivalency (FTE) for the Perfusion/Autotransfusion services required is four (4 Full Time Equivalent Employees (FTEEs) which consist of three (3) Perfusionists (2080 hrs. each) and one (1) Autotransfusionist (2080 hrs.). The number of Board Certified Perfusionists required to be on site on a daily basis is one (1)- three (3). As such, three (3) will be on site during regular business hours, while one (1) is required to be on call at all times. The contractor s employees and any subcontractors shall have all licenses, permits, and certifications as required by law and this contract. All Perfusionists and Autotransfusionist shall be certified with at least a minimum of ten (10) years of experience. All Perfusionists shall be a graduate of an accredited Allied Health Education Program School of Perfusion Technology and shall be certified by the American Board of Cardiovascular Perfusion, demonstrate evidence of meeting continuing education requirements and shall have performed a minimum of 1000 clinical cases. Experience shall be demonstrated in the following areas: Open heart surgery, autotransfusion, heart lung machine, membrane oxygenation, left/right heart bypass, intra-aortic balloon pumping and centrifugal ventricular assist device. The Authority for this procurement is the Department of Veterans Affairs Directive 1663 implementing provisions of Public Law 104-262, the Veterans Health Care Eligibility Reform Act of 1996, which expands VA s health care resources sharing authority in Title 38 United States Code (U.S.C.) Sections 8153 (sharing authority). The anticipated period of performance for this contract will be from October 1, 2020 through September 30, 2025 for the base year and four (4) - one (1) year option periods. The solicitation will be prepared in accordance with the Federal Acquisition Regulation (FAR) part 12 and evaluated in accordance with the procedures of FAR Part 13. The solicitation document will be issued as Request for Quote (RFQ) 36CVA24820Q0876. The VA anticipates issuing an solicitation for this requirement on or about July 30, 2020. Interested contractors will be responsible for downloading copies of the solicitation and (any subsequent amendments) from this website. No hard copies will be made available. The solicitation will provide instructions to contractors about the response to the Request for Quote (RFQ) submission process. Prospective quoters are encouraged to register their interest in the solicitation on the FedBizOpps (FBO) website (https://beta.sam.gov/) so that they are notified of the issuance of the solicitation and any amendments. In accordance with FAR 52.204-7, System for Award Management (SAM), prospective contractors must be registered in SAM to be eligible for award. SAM registration is required under the applicable North American Industry Classification System (NAICS) Code. Contractors may obtain SAM information at https://beta.sam.gov/. Registration is free and potential Offerors are encouraged to visit the SAM website. The NAICS Code for this procurement is 621399, Offices of All Other Miscellaneous Health Practitioners. The Product Service Code (PSC) is Q999, Other Medical Services. The Business Size Standard is $7.5 million dollars. This RFQ is a set aside for Small Business (SB).
