- THE WORK CONSISTS OF: Contractor shall furnish all labor, materials, and equipment necessary to provide janitorial services at the following locations in accordance with the Performance Work Statement:� 188 Broadway, Route 1, Saugus, MA (1,352 square feet)� 100 Cummings Center, Suite 430A, Beverly, MA (574 square feet)� 100 Independence Way, Suite F109C. Danvers, MA (2,000 square feet) 257 Andover St., Route 114, Peabody, MA (1,848 square feet)� 633 Main St., Waltham, MA {3,234 square feet)� 3-5 Washington Square, Unit lL, Haverhill, MA (2,867 square feet) 160 Winthrop Ave, Lawrence, MA {2,867 square feet)� Locations less than 3,000 square feet will require service two {2) times per week. Locations greater than 3,000 square feet will require service three (3) times per week.� THE BID SCHEDULE INCLUDED AS AN EXCEL SPREADSHEET MUST BE COMPLETED IN ITS ENTIRETY FOR QUOTES TO BE DEEMED RESPONSIVE.� THE BID SCHEDULE SHOULD BE RETURNED AS AN EXCEL SPREADSHEET TO THE MAXIMUM EXTENT PRACTICABLE AS IT IS FORMATTED TO CALCULATE ALL TOTALS FOR THE VENDOR. This procurement is set-aside for small business vendors. The applicable NAICS code is 561720 with a Small Business Size Standard of $19.5 Million.� ALL OFFERORS MUST HAVE AN ACTIVE REGISTRATION IN SAM.GOV AT THE TIME OF SUBMISSION IN ORDER TO BE CONSIDERED. ANY OFFEROR THAT DOES NOT HAVE AN ACTIVE SAM.GOV REGISTRATION WILL BE REJECTED AS NON RESPONSIVE.�
