Page 1 of 1 This notice is for Solicitation Number 36C24222R0025 - Project Number 561A4-13-106 Renovate Radiology (Site Prep) at the VA Lyons campus, NJ. The Department of Veterans Affairs is preparing to issue a solicitation (Request For Proposals-RFP) for a two-phase design-build contract to provide turnkey services necessary to design and construct the site preparation required for Project Number 561A4-13-106 Renovate Radiology (Site Prep) at the VA Lyons campus, NJ. Phase I Estimated Solicitation Date: on or about 12/20/2020 Estimated Award Date: 03/30/2021 SET-ASIDE: 100% Total Small Business (SB) NAICS: 236220 Commercial and Institutional Building Construction SMALL BUSINESS SIZE STANDARD: $39.5 million PROJECT MAGNITUDE: $2,000,000 to $5,000,000. PROJECT OVERVIEW: The project will require the selected contractor to provide all technical design and construction services, supervision, labor, materials, tools and equipment necessary for a complete engineering and construction project for site prep as described above for use in implementation of initiative. The Two-Phase Design-Build solicitation and evaluation process shall be conducted in accordance with Federal Acquisition Regulation (FAR) Subpart 36.3 (Two-Phase Design-Build Selection Procedures). The issued RFP shall set forth the requirements for responding to Phase I of the solicitation, as well as all technical evaluation factors and sub-factors for both Phase I and Phase II. At the closing date of the Phase I Solicitation, the government shall evaluate responsive proposals received in response to Phase I of the solicitation to short-list no more than three (3) contractors to Phase II. Only those successful firms selected as a result of Phase I evaluations will be requested to submit technical and pricing proposals for Phase II. Phase II proposals shall be evaluated in accordance with Phase II Evaluation Process. All inquiries that do not have the solicitation number in the subject line will not be replied to. Phone calls will not be accepted. All other details and information shall be contained in the solicitation which shall be posted on https://beta.sam.gov/. The solicitation will be released on or about 12/20/2020. The response submittal address, date and time will be included in the solicitation. All prospective bidders must be currently registered in www.sam.gov at the time of offer submission and before award of this procurement. Scope of Work: Provide turnkey (Design Build-DB) services to include all labor, materials, equipment, and supervision necessary to design and construct the site preparation needed to install a new Vantage Orian 1.5T MRI system. All work shall be performed in accordance with manufacturer s equipment drawings & specifications, VA requirement and specifications, and VA Specification 00 11 21 (Request for Proposal to Design Build). Work is located in rooms 1226 / 1227. All work shall be performed at the VA NJ Health Care System, Lyons, New Jersey 07939. Work shall comply with the applicable specifications and dimensions in the Manufacturer Preliminary Drawings and the applicable requirements set forth in Manufacturer equipment Vantage Orian 1.5T MRI system. Work-Related Services 1. Retain a set of Manufacturer's site-specific equipment installation drawings at the Site, and redline those drawings to indicate any deviations made from the requirements indicated in those drawings during the production of the Work. 2. Furnish miscellaneous shop drawings if and to the extent necessary for production of the Work. 3. Furnish an electronic copy of the record drawings showing material changes in the Work made during construction based on markup drawings and other Work changes made by the Work contractors during their performance of the Work. 4. The design for the Work will incorporate applicable building codes, rules, and regulations enacted by the state where the Site is located. General Requirements of Construction: Provide professional full time on-site supervision of the work. Hold bi-weekly construction meetings and prepare meeting minutes. Prepare and maintain a construction schedule for the work. Coordinate power and other utility shutdowns with facility representative. Coordinate materials delivery and storage with facility representative. Remove rubbish and debris as it accumulates. All debris/trash must be removed from the construction site daily. Provide required temporary facilities, including but not limited to: A. Temporary barriers/structures to secure work areas for safety purposes noise reduction and dust containment. B. Temporarily seal supply/return air diffusers to minimize contamination of ductwork and surrounding areas. Temporary lighting above ceiling during construction. Foot mats at work area entrances and exits. Infection Disease Control. Abide by facility rules and regulations in effect at the site Provide current insurance certificates Remove and legally dispose of debris. Contractor shall be required to provide the following documentation for all of his/her personnel working on VA property: Background investigation Worksheet (Attachment No. 2 under SOP Acquisition) VA Form 0711 Security and Privacy Training certification OHSA certification TB tuberculin test certification End of Description.
