- INDIAN HEALTH SERVICES (IHS) SOFTWARE-BASED MEDICAL DEVICE SUPPORT Please see the Attachments Section for the complete Request for Proposal� This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Request for Proposal (RFP) 75H70122R00003 The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2021-07. This is an unrestricted, full and open competition under NAICS Code 511210 with a size standard of $41.5 million. Issue Date: 12 October 2021 Proposal Due Date: 15 November 2021 at 2:00 P.M. pacific time or earlier due to matthew.phelps@ihs.gov. The provision at 52.212-1, Instructions to Offerors-Commercial, applies to this acquisition with addenda. The provision at 52.212-2, Evaluation-Commercial Items is included with addendum in Section M. The Contracting Officer and primary point of contact for this RFP and resulting award is Matthew Phelps.
