- SOURCES SOUGHT NOTICE Community Nursing Home Services Veteran s Healthcare System of the Ozarks Fayetteville, AR DESCRIPTION: This is a Sources Sought/Request for Information to determine the availability of potential sources having the skills and capabilities necessary to provide the appropriate level of Community Nursing Home (CNH) care to veterans and eligible beneficiaries of the Department of Veterans Affairs, Veterans Healthcare System of the Ozarks (VHSO) located at 1100 N. College Avenue, Fayetteville, AR 72703. Requirement is for Community Nursing Home Services in Newton County, AR. Five Year Ordering Period with an estimated period of performance will be from April 1, 2022 through March 31, 2027. The number of veterans requiring Community Nursing Home services (bed days) over the next five years will fluctuate and cannot be adequately determined at this time. Award of a Fixed Unit Price Indefinite Delivery Contract (IDC) with Economic Price Adjustment is anticipated. Please be aware all Federal, State and local labor laws will be applicable to this procurement including: Federal minimum wage as provided in Section 6(a)(1) of the Fair Labor Standards Act If you have specific questions regarding labor laws, you should contact your local Federal Department of Labor representative. Executive Order 14042 - Ensuring Adequate COVID Safety Protocols for Federal Contractors Federal Acquisition Regulation 52.223-99, VHA Supplemental Contract Requirements for Ensuring Adequate COVID-19 Safety Protocols for Federal Contractors. REQUIRED: Interested agencies will be Medicaid approved as applicable to the location as a Certified Nursing Home Facility and licensed as a Nursing Home by the State of Arkansas in which its provider number is based. All interested vendors are invited to provide information to contribute to this market survey/sources sought including commercial market information. THIS IS NOT A SOLICITATION ANNOUNCEMENT. This is a source sought notice. The purpose of this notice is to gain knowledge, in this order, of potential qualified sources and their size classifications relative to NAICS 623110 Nursing Care Facilities (size standard $30.0 Mil): (Service Disabled Veteran Owned/Veteran Owned Small Business (SDVOSB/VOSB), Hub zone, 8(a), small, small disadvantaged, woman owned small business, FSS/GSA contract schedule holders or large business) Responses to this synopsis will be used by the Government to make appropriate acquisition decisions. Responses to this source s sought synopsis are not considered adequate responses to the potential solicitation announcement. All interested offerors will be required to respond to the solicitation announcement in addition to responding to this source sought announcement. REQUESTED INFORMATION: STATEMENT OF CABABILITY: Submit a brief description, five (5) pages or less, that demonstrates how your company can provide the requested services at time of award. Include your company s experience in performing these services to the VA, other Government (Federal or State) agencies, or for a private facility. BUSINESS SIZE AND SOCIO-ECONOMIC STATUS: Indicate whether your business is large or small as defined by the NAICS If small, indicate if your firm qualifies as a small, emerging business, or small disadvantaged business (c) If disadvantaged, specify under which disadvantaged group and if your firm is certified under Section 8(a) of the Small Business Act Indicate if your firm is a certified Hub-zone firm Indicate if your firm is a woman-owned or operated business Indicate if your firm is a certified Service-Disabled Veteran Owned Small Business (SDVOSB) or Veteran Owned Small Business (VOSB) Include the DUNS number of your firm. In accordance with FAR 52.204-7, System for Award Management, prospective contractors must be registered in System for Award Management (SAM) and if Veteran Owned, in the VetBiz database. If applicable, VetBiz registration is required for Service-Disabled Veteran Owned Small Businesses or Veteran Owned Small Businesses) under NAICS 623110. Contractors may obtain SAM information at https://beta.sam.gov and Vet Biz information at http://www.vip.vetbiz.gov. Registration is free and potential offerors are encouraged to visit the SAM website as no IDCs can/will be awarded to any offeror not registered in SAM.
