- NOTICE OF INTENT TO SOLE SOURCE/SOURCES SOUGHT:� THIS IS NOT A NOTICE OF REQUEST FOR QUOTATION BUT INFORMATION AND PLANNING PURPOSES ONLY!� This notice does not constitute a commitment by the Government.� All information submitted in response to this announcement is voluntary and the Government will not pay for information requested nor will it compensate any respondent for any cost incurred in developing information provided to the Government. The Request for Quotation (RFQ) number is FA5606-22-Q-2002 and shall be used to reference any written responses to this source sought. Spangdahlem Air Base is contemplating a Sole Source or Brand-Name requirement to BOYD Industries, Inc. The North American Industry Classification Systems (NAICS) Code proposed is 423450 - Medical, Dental, and Hospital Equipment and Supplies Merchant Wholesalers. The size standard for this NAICS is 200 employees. The Surgery Chair will be used to perform oral surgeries to include extractions and periodontal procedures. Ideally, the item will be equipped with an aerosol collection attachment (or similar feature) that will suction any contaminated air around the provider team and lower the spread of the potential infection rate of Covid-19 and other infectious diseases. The end goal for procuring this item is to ensure the administering of quality care while also protecting both the patient and treatment team from any medical safety incidents. If interested, include in your capabilities package your DUNS, Cage Code, System for Award Management expiration date, and any related specifications/drawings. The capabilities package should explicitly demonstrate company capabilities�indicating examples of commercial sales�and product specifications related to this effort.� Also indicate if you are the manufacturer or provide the name and size of the manufacturer of the product(s) you will be supplying.� Respondents are further requested to indicate their status as a Foreign-owned/foreign-controlled firm and any contemplated use of foreign national employees on this effort. A determination by the Government not to compete based on responses to this notice is solely within the discretion of the Government. Information received will normally be considered solely for the purpose of determining whether to conduct a competitive procurement. We are interested in any size business (large or small) that can meet this requirement. Please note:� All contractors doing business with the Federal Government must be registered in the System for Award Management (SAM) database.� The website for registration is www.sam.gov.
