- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in subpart� 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is 75H71221Q00031. Requirements for Contractor to provide off-site teleradiology services to Indian Health Service, Hopi Health Care Center (HHCC), located at Highway 264 Mile Post 388, Highway 264 Mile Post 388, Polacca, AZ 86042. FAR Provision 52.212-1, Instructions to Offerors-Commercial, applies to this acquisition and any addenda to the provision. FAR Provision 52.212-2, Evaluation-Commercial Items applies and the specific evaluation criteria to be included in paragraph (a) of the provision. �Offeror who does not have SAM registration updated in the SAM.Beta.Gov will be considered non-responsive. Included in this registration is your updated Representations and Certifications.� FAR Clause 52.212-4, Contract Terms and Conditions-Commercial Items nd 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items, apply to this acquisition and a statement regarding any addenda to the clause. �Additional Clauses and Terms and Conditions are in the attached.� All quotations are due on May 27, 2022 at 12:01 PM, PST. Offers shall be based on all instructions, conditions and notices to offerors, provisions, general conditions, general requirements, and specifications contained in this solicitation. Proposals shall be submitted electronically by email to nestor.noble@ihs.gov. All correspondence and proposals should have a subject title in the email: 75H71222R00039 � Off-site Teleradiology for HHCC.� Offerors shall submit two (2) separate electronic files for the following: Price Proposal.� Complete Attachment 7 Blank Schedule of Bids Technical Proposal and Past Performance. When combine, the maximum number of pages is only eight (8).� Pages after the eighth (8th) �page will be truncated and will not be evaluated:� (A)�Technical Proposal.--�The technical proposal shall address the following:�Ability to successfully perform the Scope of Work (SOW):�Submit documentation/information to address Attachment 2 �Statement of Work (SOW). Offeror to submit documentation/information on how the SOW will be successfully provided and performed. (B)�References and Past Performance --�Submit documentation/information on past four contracts or projects of similar magnitude, Governmental or non-Governmental. �Projects should only include from year 2018 until 2021 (present) only. As a minimum, information to include name of the firm, project number and title, date or period of performance, name of point of contact: company position, email, and phone number). Technical and Cost proposals will be evaluated independently. �Do not include your price proposals for this solicitation on your Technical Proposal and Past Performance file. Each electronic file must be labeled with the Offeror's organization and the proposal (price or technical) and shall include page with which identities the name of the organization, business address, and IHS Solicitation Number (75H71221Q00031). Award will be based on the�LOWEST PRICE TECHNICALLY ACCEPTABLE proposal. CAREFULLY follow the instructions contained herein.�
