- Special Notice Special Notice Page 4 of 4 Page 4 of 4 Special Notice *= Required Field Special Notice Page 1 of 3 Under Statutory Authority 40 U.S.C. 501 and 41 U.S.C. 251 the VA Great Lakes Acquisition Center (GLAC) intends to negotiate a sole source contract with PROAIM AMERICA S LLC. VISN 12 is pursuing a resource saving approach by providing a VISN service contract that covers the support, maintenance, and licensing of existing Zeiss FORUM server platform at all its eight (8) VA Medical Centers (VAMC) within the VISN. The VAMCs are Milwaukee, Tomah, Hines, Iron Mountain, Madison, Danville, Jesse Brown, and North Chicago FHCC. The contractor shall provide all necessary professional services, software maintenance, interface support, phone technical support, remote support, and software upgrades including bug fixes to existing VA-owned Zeiss FORUM servers with the Zeiss FORUM Viewer for all 8 sites in VISN 12 through a service contract with a base period of one (1) year and 4 option-year periods of performance that are exercisable at the discretion of the government Background: VISN 12 has implemented a Zeiss FORUM platform as an Eye PACS (Picture Archiving and Communication System) for all of its 8 VAMCs to manage the transmission of eye images from the various eye clinic modalities into the EMR (Electronic Medical Records) of record. The original purchase of the Zeiss FORUM platform has come with a warranty, which will be expiring on 9/30/2025. This request is to award a base plus 4 option year service contract to continue maintenance coverage of the platform and provide necessary software upgrades. This acquisition is conducted under the authority of 40 U.S.C. 501 and 41 U.S.C. 251 implemented in FAR 8.405-6, only one responsible source and no other supplies or services will satisfy agency requirements. This will be a firm fixed price contract for a BASE + 4 OPTION YEAR PERIODS of 12 months each. The NAICS Code is 811210. This Notice of Intent is not a request for competitive quotes. No solicitation documents are available, and telephone requests will not be honored. However, if a firm believes it can meet the requirements it must furnish information about its products and services, as well as references from other customers who are using these products and service to the Contracting Officer by 10:00AM (CST) 06/11/2025. Supporting evidence must be furnished in sufficient detail to demonstrate the ability to comply with the above requirements. Responses received will be evaluated however; a determination by the Government not to compete the proposed procurement based upon responses to this notice is solely within the discretion of the Government. If no responses are received, the Contracting Officer will proceed with the sole source negotiation with the Proaim America s LLC.
