- 36C24625Q0738 Post Site Visit Questions Can the government provide historical data for the past three years for the number of retests required at Fayetteville VAMC? The Government will not provide historical data for the retests. Can the government provide historical data for the past three years for the number of retests required at Wilmington VA Clinic? The Government will not provide historical data for the retests. Can the government indicate what percent of the total number of retests allotted in item number 0003 could be expected for each location? We cannot predict that because we do not know if the lines would fail or pass until results come back from the Lab. If samples are taken 2nd week of every month, this would reduce number of retest incase we fail because we can always pick chairs that failed and add to the new rooms that needs to be tested. Would the government consider separating the NTE for retests into separate CLINS for each location and an approximation of retests for each location? No, we could not do that because the 2 clinics are on same contract. The start date for the solicitation is 16 JUNE 2025, can the government confirm if collections will be needed for June at both locations? Yes. Collections will be needed at both locations. Section 3 of E.1 Instructions to Offerors shows 3 Sections to submit, Offer Form, Pricing, and Representations and Certifications to be submitted then in Section 6 there are 3 Factors listed, can the government clarify that all Factors listed in Section 6 must be submitted and is it acceptable to provide all factors in one document or does the government require 3 separate documents? All factors in section 6 must be submitted, with the only exception being factor 2: past performance. As per part of factor 2 s description: Quoters with no relevant past performance or whom information is not available, shall not be evaluated favorably or unfavorably on past performance, but may be considered less favorably than a quoter with favorable performance history. It is not required, but it is recommended that past performance be submitted. All factors can be submitted in one document, or in three (3) separate documents. Could you please confirm whether any specific certifications, documentation, or approvals are required for the technician to perform the sampling portion of this contract? Or is relevant experience sufficient? Contractor needs to train technician on proper way to collect samples according to Lab specification Did the previous contractor use SM 9215A? Is that a requirement or can SM 9215B or 9215E also be used? Standard method HPC 9215A
