- Page 3 of 3 THIS REQUEST FOR INFORMATION IS ISSUED SOLELY FOR INFORMATION AND PLANNING PURPOSES ONLY AND DOES NOT CONSTITUTE A SOLICITATION. This is a Request for Information (RFI)/Sources Sought Notice (SS), this is not a Request for Quotes (RFQ) or Request for Proposals (RFP). No formal solicitation document exists at this present time. This is not a solicitation announcement. The purpose of this Sources Sought notice NAICS 339112 surgical manufacturing PSC 6530 surgical supplies NAIC size standard of 1000 employees for the Beckley VAMC. Responses to this Sources Sought notice will be utilized by the Government to make appropriate acquisition decisions. After reviewing the responses to this Sources Sought notice, a solicitation announcement may or may not be published in SEWP, SAM.gov (SAM) or GSA ebuy. REQUESTED INFORMATION If you are interested and capable, and can provide the required supply, please provide the requested information for questions 1-12 as indicated below: Responses to this Sources Sought notice shall also include: Provide interested vendor company name, SAM UEI, address, point of contact. Provide interested vendor's business size status to include representations NAIC and PSC code? Does vendor have capability, product, and interest of an end item product listed in general information? Provide detailed responses to equipment general information listed below. Provide product end item info and price: product name/model/ model number/ product brochure/ specification sheet/warranty/life expectancy/solution/maintenance. Where is end item product place of manufacturer? Is interested vendor an authorized distributor of a domestic end item product? Is interested vendor a manufacturer of a domestic end item product? Is interested vendor a manufacturer of a nondomestic end item product? Is interested vendor an authorized distributor of a nondomestic end item product? If authorized distributor, what is manufacturer size? Provide an authorized distributor letter if vendor is not the manufacturer. SOW A. GENERAL INFORMATION High level disinfectant probe disinfector equipment requirement (yes/no/NA with details): Touchscreen operating panel- No rinse Automated system enclosed high-level disinfection of clean and dry probes - Minimized chemical use on equipment for HLD- chemical disinfection cycle time for 1 cleaned and dried probe - Automated digital cleaning cycle logging for streamlined tracking of log reports Temperature setting capable if solution required- Chemical disposal requirements- Small footprint- Accessories Printer- Accessories Paper- Power cord- 110 volts- Accessories Wall mounts- Accessories Positioner - Accessories Probe covers - Chemical cartridge/solution disposal requirement - Equipment life expectancy- If software is required provide software name, and VA OIT validation number- Provided preventive maintenance requirement & manual- May or may not require a LAN connection- Provide FDA approval number- NOTES All Capability Statements/Responses to this Sources Sought notice shall be submitted via ebuy Telephone, Fax, or any other form of communication inquiries will not be reviewed or accepted in response to this Sources Sought notice. Responses to this Sources Sought notice must be received no later than 10/31/25 at 12:00 p.m. (Eastern Standard Time). This Sources Sought notice is to assist the VA in determines sources only. A solicitation is not currently available. If a solicitation is issued, it may or may not be announced at a later date, and all interested parties shall respond to that solicitation announcement separately from the responses to this Sources Sought notice.
