Description

Amendment 2: The offers due date is extended to 01/05/2026. 1. The purpose of this solicitation amendment is to: a. Respond to the questions received. b. Revise the SOW. c. Add Attachments. d. Extend the offers due date. 2. The Consolidated Responses Q&A Zuni Roof Replacement are included with ""B05-A02 QA Offers-Due Date Zuni Roof Replacement and Fall Protection Construction"". 3. Attachment J01 SOW ZRRF is replaced with the November 2025 version and is now titled J01 SOW ZRRF Nov2025. 4. Attachment J09 ZIH Re-Roof Plans 2005 is added. This is a zip file of the plans for the current roof. 5. The offers due date is extended to 01/05/2026. 6. Acknowledge this and all amendments on block 19 of the SF-1442. 7. All other terms and conditions remain unchanged. Amendment 1: The offers due date is extended to 12/16/2025 to allow time to publish the answers to the questions received in response to the solicitation. Added the SF-30 Amendment of Solicitation form named ""B05-A01 Amendment to Offers-Due Date Zuni Roof Replacement and Fall Protection Construction"". Remember to acknowledge this and all amendments with your offer. RFP Title: Zuni Roof Replacement and Fall Protection Construction Period of Performance: 180 days. Construction Magnitude: magnitude of this construction project is anticipated to be between $1,000,000 and $5,000,000. Contract Type: FFP Goal: This project will provide for the needed replacement of the roofing systems at the Zuni Comprehensive Community Health Center; and to include a fall protection system. Site Visit & Preproposal Conference: Refer to L.5 (page 51 of RFP Zuni Roof Replace-Fall Protect) Including the RFP, there are nine (9) attachments.
